1. The residence application form prepared for this, signed by the previous sponsor with the waiver and the new sponsor with approval
2. Two (2) personal photos with a blue background measuring 4×6.
3. The original passport of the sponsored person.
4. The original civil ID of the sponsored (for cancellation).
5. The original civil ID of the new sponsor and a copy of it.
6. The original civil ID of the assigning sponsor and a copy of it.
7. Health insurance certificate.
