- Copy of experience certificates
- Record summary
- An acquaintance document
- Barcode document for security
- Good conduct certificate
- Profession license
- Medical error insurance policy
- Notification of appointment
- Job nomination letter
- A record of operations for the last two years " is required from the surgeon"
- Personal photo
- Health benefits for the last two years for doctors nominated for exemption from the Health Authority exam
- Register of subjects or specialty program you have.
