How to get insurance continuity certificate?

According to the Health Insurance Act No. 23/2005 and its Executive Regulations No. 25/2006, the employer and / or the sponsor in the health insurance program shall not participate or be residing under his sponsorship, and a fine of 300 dirhams per month for everyone who is not subscribed for.
