NEW (Re-NEW) PASSPORT REQUIREMENTS:

1. Passport Application Form fully completed and signed (Fill 02 copies)
2. A lost report of your current passport
3. Two passport size pictures
4. Payment of the fees
5. Two copies of your birth certificate both sides.
6. Original Marriage certificate for all married ladies or Original divorce certificate/divorce decree for women who wish to change their names. & 02 copies
