Epididymal cysts
Epididymal cysts are among the most common epidydimal problems that present as palpable soft mass at the base of  the scrotum or the tip of testis, its usually filled with clear serous fluid or spermatozoa, and its variable in size and number as you can have more than one cyst in 30 % of cases, its usually asymptomatic but sometimes it causes  pain, swelling, or redness or a feeling of pressure at the base of the penis. its  not usually dangerous and treated only when  cause pain or  when they decrease the blood supply to the penis (rare). Treatment is not usually needed except if it  gets larger or causes discomfort, then the urologist will decide a procedure to remove it surgically.
Required tests 
To diagnose the actual character of the cyst you should do a clinical exam and perform semen analysis, testosterone level , cbc to rule out infection and ultrasound exam of the testis to confirm the diagnosis.

Our advice
You are adviced to do regular exercise, avoid long standing, and avoid wearing tight fitting pants and underwear and consult your treating urologist if symptomes perisist or the lump is getting larger 

Valvular heart repair surgeries 
The procedure of opern surgical intervention for repair of valvular heart disease is considered a major surgery where the patient is put on an alternative heart machine to pump his blood and the heart will be paralysed during the surgery to facilitate the repair procedure. Beside from the surgical procedure the main determinant of the out come of the surgical repair procedure is the general status of the patient and  which valve is affected, for how long , degree of severity and complications. The overall mortality recognized rate for most vavular heart regurgitation repair surgery is variable from 3-5% depending on many variables.
required investigations preoperatively 
To do risk assessment preoperative a complete check for the general patient status should be done upon reviewing the patient general condition with proper clinical exam and doing ECG, cardiac enzymes CK, CK MB , and assessment of the coronary arteries that supply the heart using cardiac cath of nessessary, a review of the lung funtion using lung funtion tests especially in smoker patients and measurment of lung hypertension using echocardiagraphy is necessary, a check for kidney function Urea and Creatinin, liver function AST , ALT , TOTAL PROTEIN , ALBUMIN and electrolytes K, Na , Ca, and a check for patient coagulation profile PT, PTT, INR.
High risk patients 

• Severe valvular compromise and congestive heart failure 
• pulmory hypertension 
• ischmic heart disease 
• severe carotid artery stenosis that can result in stroke 
• Significant kidney or liver damage 
• Coagulation disease
