Hello, my 14 year old son sometimes has issues with the tip of his fingers and toes where the skin becomes very red and sensitive. He usually uses a cream for a week and it goes away for a while. He has been swimming lately and it has gotten worse and the cream is not effective. What can we do?

Management and sympromes of Raynaud's phenomenon

Raynauds phenomen is one of diseases that affect arterial structures of extremitites life tip of fingers and toes as it cause spasm and discoloration, pain and parasthesia of fingers and toes that can progress in some situation to a state of tissue loss if it was more severe, most cases of raynaud phenomenon are considered to be idiopathic without underlying cause especially if it was happening symmetrically of both hands but a set of investigations must be taken to uncover any underlying Rheumatologic diseases. and to treat the acute attack of vasospasm its recommended to warm the hand and avoid cold water and cold weather as it aggrevate the state of vasospasm, on the other hand the use of Nifidipine 30 mg tab once daily along with Nitroglycerin 1% oint twice daily along the use of topical L.Arginin containing oint twice daily  will improve the symptomes, and if the treatment failed to make any progress its wise to consult your treating fdamily doctor.
Required investigations 
The phenomena is diagnosed clinically and the investigations are done to uncover any underlying disease, CBC can be done to look for significant rise in Hgb above 15.5 mg/dl and Platelet. kidney function Urea+ creatinin to look for signs of jidney impairment, Coagulation panel of PT,PTT to look for any evidence of hypercoagulable state, Thyroid Functions Tests TSH, T4, T3 to look for any signs of hypothyroidism, Rheumatologic panel tests like ESR, ANA, RF, Anti cardiolepin and serum protein.
some imaging studies can be used to confirm the state of vasospasm like thermography and arrteriography.
causes of Raynaud phenomena 
  • idiopathic 
  • scleroderma 
  • connective tissue dieases and Rheumatoid arthritits 
  • systemic lupus erythmatosis 
  • Sjogren syndrome 
  • other vasculitits 
  • mycoplasm bacteria infection 

our advice
 The patient should avoid contact with cold substances and cold water and to do rewarming of the hand once the symptomes appeared in conjucntion with taking the appropriate medications and consult your family doctor if treatment failed to meake any progress.
