the treatment is by metformin as the first line treatment by lowering the production of glucose from the liver then a drugs that cause your pancreas to over prduce insulin and by adrugs that make your kidney release the glucose in the urine such as dapagliflozin and in some severe cases the insulin is given on its many different forms ( long, short, ultra long  and mid acting insulin )
how can i reduce the level of random blood suger ?
there is an estemation for the additional insulin units needed ( now we are talking about the regular insulin ) each 1 unit of insulin ( 100 unit per ml concentation ) cause a drop of glucose by 50 mg/dl 
the normal glucose level in this patient before meals should be 125 mg/dl or less if the patient is fasting 
what should be done ?
you should visit the endocrinologist for the right diagnosis and the correct treatment and to regulate the levels of insulin needed daily based upon on the A1C test
