this pain maybe due to torsion so you need to surgical correction or manual procdure by doctors also maybe due to inflamation need levofloxacin 500 mg twice daily in addition to it need ibuprofen 500 mg twice daily and cold bath , may be due to stone and this pain referd pain need antibiotis and renal s to increase flow twice daily and drink water probaly through out day

semen analysis

its appear as pain decrease with elevation testes and redness and swelling on it also fever and nausea and vomitting also pain on micturation
