Dr hope you doing well as Iam 38 years old man have 20 to 28 RBCs in urine test and 3+ protein. Before 3 years dr did cyscostopy also every thing fine beacuse of Rbc presist so i went i don't have any pain in kidney or feelings but proteins I can see urine as alot of foamings in the urine my working environment is offshore 28 days on and off 28 days can you understand my proteins issue and rbcs are constant 20 to 30 Regards, Mukhtiar Hussain Dubai.

What we advice you ?
you mostly complain from kidney disease even if you dont feel pain now but yo may complain in the future so 1st step to do is kidney biobsy to exclude out autoimmune disease causing high level for protien and RBC in urine as you may complain from:
- glomuronephrities 
- pyelnephrities 
- good pauster disease 
- nephrotic sundrome 
- nephritic disease 
- basement membrane disease 
- graneulomanteus disease 
test what should you do now ?
- renal biobsy 
- urine analysies 
- urine culture 
- renal ultrasoound 
- isotop scan 
and for mangment ?
- it depend pon the cause but generally 
- you need to take prednisolone 5 mg daily 
- ciproflaxcin 500 twice daily 
- rawatenex 4 times daily 
- colliurinal twice daily 
- biological therapy 
- immunosuppresant therapy 
- drink plenty of water 
- immmunglobiulin injection

Great & prompt response dr really, Is it prostate issue can be? while urinating feeling little burnings and while intercourse/ Discharging time feeling burnings also. Regarding reports urine culture no growth. Creatinine report 0.8 mg/dl (previous 1.0 mg/dl) eGFR greater than 60ml/min/1.73m2. Ultrasound also ok. Cyscostopy +bilateral RPG+ bladder biopsy results as below Features are suggestive of polypoid cystitis Owing to increase number of mast cells Interstitial cystitis is also considered .Urine reports, Protein 1.5g/l 3+ sugar normal, ketones negative, bacteria few, epith cells 2 , Mucus positive, granular cast 03 LPF.  if ultrasound normal we ca eclude out prostate issue an prostate dont cause high urine protien but for polypoid cystitis and nterstitial cystitis could be in which need surgica l excision mostly if cystoscopy prove that

Definitely iam confused completely what is that you mentioned surgical excision I have go for that? Is it any procedure? Yes main issue is protein high and Red blood cells from where they are coming? Advise someone tablets for me and path forward who I have to go for check are you understanding for need kidney biopsy?? Is it chance to RBC and protein come from interstitial cyclists ? But I don't have any pain in bladder yes urinating many times In a day we go for cyscostopy again or go for kidney biopsy better please advise either try some medications?Regards, Mukhtiar Hussain Dubai petroleum establishment

well it is rearly from acute intrstial but in chroni case yes it came 
and polypoid cause this too

Dr,Can you send medications to control this With prescription dear as I can purchase antibiotics medicine by my aentna insurance card I am agreed I believe because cyscostopy and polipod issue in bladder can cause protein?? Agreed can be leak red blood cells but how protein? So advise medicine please kidney biopsy so risky better to treat cystitis .Thanks good study dr MahmoudRegards,

Morning Dr ,Yes , but IC is not chronic as no any pain are feelings, more attention is required on proteins 3+ and Red blood cells so what you advising me to go for kidney biopsy?? Or cyscostopy again or other procedure please?

1STLY I advised you to renal bibsy to exclude out any inflamatory process in kidney 
but urinary bladder polyposies can cause symptoms like this

1. Polyposies can cause RBC in urine or proteinuria?2. Now I can see protein in my urine and lots of white foaming currently iam taking Rowatinex capsules + Saw Palmetto 3. Can I use Ciprofloxacin antibiotics with above medicines. If continue proteins increases what I do ?

polyposies can cause protienuria and heamturia 
ther is no need to take ciproflaxcin now 
One way to remove polyps is with a type of surgery called transurethral bladder resection (TURBT). The doctor first puts a cystoscope through your urethra into your bladder. Then, the surgeon uses a laser, a wire loop, or electricity to remove the growth.
but for small amounts of protien and blood you dont want to 
but i remeber fo the necessary to do renal biobsy as may the problems from them ...

MAHMOUD, how could I have send bladder biopsy, urine culture test and creatinine as you can understand better for 24 hrs urine amount 2100Ml protein were found 2410 is is leading to bit higher ? Please let me know how to send that tests as well can take decisions for renal biopsy or bladder surgery please
