The patient shall obtain the treatment visa by applying to the Canadian embassy or the authority concerned with issuing the visas, provided that to attach a valid passport, the approval of the medical authority, approval of the travel of the companion, hospital reservation and flight tickets, photograph, and pay the visa fees.
