Please I need your help about poland visa

Kindly be noted that if you are in the UAE and you wish to obtain a visa to Poland, you have to go to the Embassy of Poland in the UAE and provide the following: 
First, you have to download the Polish visa application form, then you have to complete the visa application form and submit it in the corresponding spaces. You can also fill out the Schengen Visa application form online and print a copy of the Schengen visa application form
A copy of your valid passport for at least three months
Two personal photos (White background)
Bank Statement active for the recent three months before submitting the application
Air ticket (Go & Away)
Travel health insurance policy, which must cover any medical emergency cases with hospital care and travel to their country of origin due to medical reasons, which must be insured. Knowing that,
 this health insurance policy must cover expenses up to 30,000 euros.
Cover letter explaining the purpose of the visit to Poland and the itinerary.
Book a round-trip flight or itinerary with flight dates and numbers that determine entry and exit from Poland.
Proof of residence for the duration of the stay in Poland.
Evidence of sufficient financial means for the period of stay in Poland.
