My sister is 17 years old. She is having high fever vomiting abdominal pain for more then one week. Doctor did his blood ultrasound and blood tests which shows acute cholycystitis and her Total bilirubin 6, Direct bilirubin 4.2 Sgpt 414 Alkphos 330 Her jaundice is getting worse day by day and still the same intensity of abdominal pain. The doctor is unable to diagnose the cause. Kindly help me regarding this case what should we do and what could be the diagnosis? I'll be really thankful to you.

Treatment

Mostly your sister suffers from what's called obstructive jaundice , which is a condition resulting from obstruction of the biliary tree

Obstruction can occur due to many reasons , but regarding the scenario you provide and the ultrasound results, this is mostly due stone slippage from the cystic duct which is a small tube like structure connecting the gallbladder with the biliary tree

This issue must be addressed urgently because it may lead to sepsis which results in disturbances in the vital signs and systematic toxicity

This is usually addressed by doing ERCP with an endoscopic extraction of the stones through the duodenum , and through hospitalization she should be covered with strong antibiotics and intravenous fluid and removal the gallbladder when getting better

Sometimes the condition is so severe that would require ICU admission

Diagnosis

This is usually done by physical exam and history taking and doing lab investigation like which you mentioned before, in addition to the US and ERCP and sometimes MRCP

 Signs and symptoms

-jaundice

- fever

- abdominal pain

- disturbance in vital signs

- affection of mentality

- dark urine

- itching

- light stool

- others

Thank you so much for the reply. But her US results shows that there are no stones and there is inflammatory changes in liver and cholycystitis. Also she is suffering from typhoid fever which I forget to mention.

Well typhoid fever as long case complication can cause cholycystities which need surgical excision and intravenous antibiotic
