I want to file the case in lobour court

According to the law, the case must include several documents, including:
-Personal details, addresses of the plaintiff, and defendant
-The case subject
-Requests and their reasons
-The case submitted date’s to the Case Management Office
-The court where the lawsuit was filed
-Signature of the claimant or his representative
