my brothers Blood platelet count is 121000 is it...

 

 

Generally speaking , no its not , as long as your brother is healthy and asymptomatic and has no serious underplaying and a progressive disease.  But this low count should be monitored by hematologist.

 

Most of the cases are actually self limited, unless the  patient develop severe or long-term (chronic) low platelet count , doctors usually target the cause, for example if it caused by a drug like heparin it should be stopped , sometimes  patient may need platelet transfusion if his count is low significantly (below 20-30K) or causes symptoms , and transfusion may be needed if any surgery is expected with low count (below 70K) or he may need surgery for removal of the spleen since it may cause the platelet destruction , in other cases of immune induced platelet destruction doctors may prescribe corticosteroid or drugs that slow down the immune  system.

It usually starts by physical exam and history taking, for example doctors may investigate any recent respiratory infection or any drug taken recently , then he may proceeds with lab investigation like CBC which usually includes platelet count and if its below than 150K it recognized a low count. Doctors also order Ultrasound of the abdomen, which may gives clues to the cause to the underlying  cause. In cases of hematological malignancies biopsies maybe taken as well from the enlarged lymph node or bone marrow for example  

Symptoms and signs
Easy bruising of the skin and mucus membranes in legs arms gingival mucosa- 
Difficult to control  bleeding from injuries- 
Bleeding from nose- 
Blood in urine or stools and can be noticed by change in color to red- 
heavy menses in ladies- 
Fatigue- 
Large  spleen- 
Yellowish discoloration of skin or sclera- 

It usually due to 2 main cuases- 

Increased distruction or decrease production

Leukemia- 
Pregnancy- 
Drugs- 
Immune induced destruction like ITP- 
Some types of anemia- 
Viral infections, such as hepatitis C or HIV- 
Chemotherapy drugs- 
alcohol consumption- 
infection or sepsis-
