-Personally identifiable information for the person who want to work within the country
-Provide the travel document number and the name of the authority issuing the document
-Identification statement from the future employer as well as information about the working hours in the country
-Provide a copy of the travel document containing the basic identification data of the person who want to work
-A declaration from employer that he hires foreign workers
-A documented and officially certified copy of the professional ability of the foreigner working in the Czech territory such as professional certificate, secondary certificate, diploma and other certificates.
-A valid passport
-Personal image with a white background
