We would like to inform you that you shall contact the Department of Health in your municipality and apply for the health card with the following documents:

* Copy of the store license.

* Copy of the certificate of affiliation in Chamber of Commerce and Industry

* Copy of the labor card if you are an expatriate, and a copy of the ID card if you are Omani.

* A medical examination for the Omani worker, while the expatriate will be satisfied with a copy of the labor card.

* Two photographs
