my Facebook account is disabled how to enable

Follow the steps below to restore the disabled account

 Please click on the following link facebook.help
  Enter the email address of the disabled account
  The name of the Facebook account as it exists in the disabled account
  Upload your profile photo by clicking on choose files and selecting the image from your phone or computer
  Write additional information about you in the box shown in the image
  Click Send
  After sending the request you have to wait for the response by the Facebook administration
