Firstly you need to calm and then call for help, take your father to emergency room, let doctors to examine him and if there is a serious problem then doctors will manage it. Blood pressure is high but not serious and there is no poor consequences that might follow systolic BP of 150 so you might manage this BP with only relaxation of your father and drink water and try to sleep. Sugar level is high and it seems to me that he has diabetes, if not then he might has it but never diagnosed with it. He will receive treatment in ER, he needs to take insulin and fluid and to be under observation..
